How would you describe your time at Developmental Services of S...?
The new position, I had was with four clients in the Day Program. My duties was to document five days a week on all four, which included their skills acquisition data that had Task, Training Cues, and Response Codes. All of these task had to do with verbal cues, how they performed them, and any response, such as loud outbursts, or just not wanting to do anything for that day. All documents had to be signed by my supervisor and dated. Monthly progress notes, showing their progress on whatever objective was for that individual, and comments on whether the person could do better. One persons Formal Objective was Anna (not her name) By 3/95, Anna will indpendently check and make any necessary improvements in her appearence before going out in the community, 100% of the time for two (2) consecutive months. Currect Level of Performance: Anna needs verbal cues to change soiled clothing, personal hygiene, and dressing for the weather. At times Anna refuses to make any changes. Her review is done on a monthly basis, and her primary is the Community Resource Developer, Somersworth Community Office Staff Objective: Staff will encourage Anna to use appropriate social skills while in the community. The community resource developer will report to Case Management on a quarterly basis. There was always a deadline for each person to acheive their objectives. on each persons objective implementation sheet, there are five list of things to inprove on before her deadline is up. Each person has their own Individual Service Plan/Placement Approval Form , with their guardian's approval. Each one has to sign a Media Release Form for a Tape/Video/Other, such as photographs, date, and agreeing that this will only be used for this purpose. Staff have Individual Service Plan Review meeting to talk each persons formal objective, and they are graded if changes have to be made, they are made, giving them another Formal Objective B, along with a ISP Review Face Sheet. Each one also has a Weekly Schedule from 8am to 4:00pm....

I did total patient care, including bed baths, tub baths, reporting intakes and outputs,BM's TPR's along with BP's. Documenting any new information on patients including falls, skin tears, or bed sores and report it to the RN on duty for that shift.
