RESPONSIBILITY:

 

Works under the supervision of the Manager.  The Clinical Coordinator has responsibility for the coordination of patient care and shift activities on the unit/ department.  As a Registered Nurse, the Clinical Coordinator assesses, plans, implements and evaluates the coordination of individual patient care on a nursing unit according to unit policies and procedures. Supports and upholds the Mission , Vision, Values and Guiding Behaviors, Patient Bill of Rights and the Code of Business and Ethical conduct.

 

JOB DUTIES:

 

1.             Leads the patient care team, through delegation and provision of direct and indirect patient care.  Functions as an expert/Role Model practitioner for a defined group of patients by analyzing nursing care and identifying clinical issues.  Coordinates complex care across the continuum of care from admission, transfer and discharge.

2              Ensures the provision of education to parents including diagnosis-related discharge planning and wellness training (parenting skills).

3              Leads by example and observes the clinical competence and behavior of staff and provides feedback to the manager.

5.             Supports departmental goals and the hospital strategic plan.

6.             Reviews completion of assessments of patients utilizing knowledge of age specific criteria: develops plan of care based upon assessment and provide immediate feedback. Assures that staff implements and documents the plan of care, evaluate the patient’s response to care, and updates plan of care.  Participates in discharge planning; provides instructions to patient/family.

7.             Oversees the shift activities of the respective department to exceed and assure customer delight (patients, families, physicians).

8.            Rounds with the physicians and visits all patients to ensure quality and family –directed care delivery.

9.             Promotes, respects, supports and collaborates within the multidisciplinary teams while demonstrating the

                MCH way/

10.           Participates in unit rounds and discharge planning; provides instruction to patient/family.

11.           Promotes/practices cost containment by flexing staffing when appropriate by utilizing resources and by

                maintaining/ ordering appropriate supplies.

12.           Coordinate patients flow to assure efficiency and attends the bed meetings.

13.           Maintains the dignity and confidentiality of the patient; serves as patient advocate.

14.           Promotes safety standards in order to ensure an error free environment.

15.           Performs open chart reviews/data collection and shares findings with leadership team.

16.           Updates intelli-chart daily and flex staffing accordingly.

17.           Applies knowledge of growth and development in customer interactions and treatment.

18.           Coordinates pre-shift team huddles, sharing timely information and communication with care team.

19.           Supports the department’s performance improvement changes to drive customer delight, clinical quality

                care and financial performance.

 

20.           Ensures that all regulatory standards are in compliance as to: completion of medication reconciliation, hand off, verbal/telephone orders, and accurate use of abbreviation

EDUCATION:

Nursing Degree from an accredited school of nursing.  A BSN or BS in a health care field is preferred.

EXPERIENCE:

 Two years pediatric nursing including charge experience is preferred.  Managerial experience preferred.

LICENSE:  

RN License to practice professional nursing in the state of Florida

 

PHYSICAL DEMANDS/MENTAL DEMANDS:

Physical Demands: Requires standing and walking for extended periods of time, the ability to move within and between rooms where patients are treated. Requires the ability lift 50 pounds under ideal conditions in order to position, transfer, and ambulate patients. Requires the ability to kneel to the floor and the ability to push 10 pounds frequently and 20 pounds occasionally. Precise hand eye coordination and finger dexterity is required to administer medications and treatments. Requires the ability to visually distinguish letters and symbols and visually assess the patient. Must be able to don personal protective equipment.

                Mental Demands:  Requires the ability to use logical and scientific thinking to interpret technical data and solve a broad range of problems. Requires the ability to use complex mathematical techniques to budget and provide productivity assessments. Requires the ability to interpret, adapt and react calmly under stressful conditions. Must be able to relate cooperatively and constructively with clients and co-workers and effectively monitor and develop the abilities of subordinates. Requires the ability to communicate verbally and in writing and to maintain confidentiality of sensitive information.
