How would you describe your time at Alzheimer's Living Center a...?

??? Responsible for the administration of the Social Service Department within policies and procedures of the organization for a caseload of 99 residents with Alzheimer???s disease/Dementia or memory impairment.
??? Developed a new initial assessment, comprehensive social history and
psychosocial assessment, and completed the MDS (Minimum Data Set) with updated care plans for all residents.
??? Documented including daily, monthly, and quarterly notes or changes in condition while monitoring participants overall progress or decline relating to their unique psychosocial issues within the policy time frame.
??? Assisted residents and families with adjustments to change; which included orienting to facility setting and reviewed resident rights.
??? Resolved all grievances from families in the facility with a written report to the Administrator for follow-up.
??? Reported unusual occurrences to the Ombudsman and accurately made a written report within two (2) working days of the event using the standard reporting form, SOC 341, "Report of Suspected Elder/Dependant Adult Abuse".
??? Participated consistently in the Interdisciplary Team Care Plan, Medicare Utilization, Stand up, and Falls Review on a daily basis reducing the overall psychotropic and restraint use each quarter.
